Atlantic Health is seeking a Nurse Navigator for the Weight Management Program. The Nurse Navigator is a professional Registered Nurse who provides individualized navigation and coordination for patients accessing weight management services across Atlantic Health. This role serves as a centralized point of access for patients and referring providers, guiding patients to the most appropriate services, including Metabolic Medicine, Bariatric Surgery, Medical Weight Management, Nutrition Services, Behavioral Health Services, and other related programs.
Using the nursing process, the Nurse Navigator supports timely access to care, removes barriers to treatment, and enhances the patient experience across the continuum of weight management care. The Nurse Navigator collaborates closely with multidisciplinary clinical and administrative teams to streamline workflows, improve referral processes, and promote coordinated, patient-centered care.
Principal Accountabilities
- Serve as the primary point of contact for patients seeking weight management services; answer inbound calls, initiate outbound follow-up, and respond to patient inquiries in a timely manner.
- Assess patient needs, readiness, and goals to guide navigation to appropriate weight management services and care pathways.
- Provide patient-centered education regarding treatment options, program requirements, and expectations across weight management services.
- Coordinate and schedule consultations, appointments, diagnostic testing, procedures, and follow-up visits across multiple departments.
- Identify actual and potential barriers to care, including insurance coverage, financial concerns, access issues, or health literacy challenges, and facilitate appropriate referrals or interventions.
- Collaborate with multidisciplinary teams including physicians, advanced practice providers, dietitians, behavioral health clinicians, and program leadership to ensure coordinated, efficient care.
- Support smooth transitions between services (e.g., medical weight management to bariatric surgery or metabolic medicine).
- Assist with insurance verification, referrals, and prior authorizations as needed to facilitate access to services.
- Serve as a liaison between patients, families, providers, and departments to enhance communication and continuity of care.
- Participate in workflow development, referral pathway optimization, and process improvement initiatives related to weight management services.
Travel between Atlantic Health sites as required to support program operations, build relationships with clinical teams, and maintain familiarity with services offered across locations.
Document patient interactions, navigation activities, barriers, interventions, and outcomes accurately within the electronic medical record (EMR).
Participate in meetings, quality improvement initiatives, and data collection to support program outcomes and performance metrics.
- Maintain confidentiality, demonstrate cultural sensitivity, and act as a patient advocate at all times.
- Complete required education, training, and compliance activities in accordance with organizational and regulatory requirements.
- Perform other duties as assigned.
Required
- Active Registered Nurse (RN) license in the State of New Jersey.
- Graduation from an accredited nursing program.
- Bachelor of Science in Nursing (BSN).
- 3-5 years of clinical nursing experience.
- Demonstrated ability to work independently, exercise sound clinical judgment, and manage multiple priorities in a fast-paced environment.
- Excellent verbal and written communication, organizational, and interpersonal skills.
- Working knowledge of electronic medical records (EMR) systems. EPIC preferred.
- Proficiency with Microsoft Office Suite (Word, Excel, Outlook).
Preferred
- Experience in care coordination, patient navigation, chronic disease management, bariatrics, metabolic medicine, primary care, or population health.
- Knowledge of insurance processes, referrals, and prior authorization workflows.
- Experience participating in quality improvement or workflow optimization initiatives
