Job Description:
Collaborates with a multidisciplinary team to develop and implement individualized care plans. Maintains thorough and confidential patient charts and records, documenting status, interventions, and outcomes accurately. Facilitates communication and processes regarding UNOS listing, organ placement, and recipient admission. Arranges for diagnostic and therapeutic services, as well as consultations for patients. Provides direct care to transplant patients in both inpatient and outpatient settings, ensuring continuity of care. Adapts to rapidly changing technologies, policies, and workload through critical thinking and flexibility. Assesses patients’ compliance and ability to adhere to medication regimens and treatment recommendations. Conducts pre-transplant interviews with prospective patients and families, interpreting eligibility criteria. Prepares and analyzes pre-transplant clinical data, collaborating with medical personnel for patient and family preparation. Implements patient care plans and observes outcomes in accordance with nursing standards and physician direction. Coordinates all aspects of the transplant process, including pre-transplant preparations and post-transplant follow-up. Educates patients and families on all aspects of transplantation, documenting their understanding.
